Please call or email with your Credit Card information ( Card Number, Expiration date, Security Code, Billing Address and Name as it appears on the Card) in the space above. Once approved, Arthomed will FedEx the product to the Healthcare Provider administering the product. Please call if you have any questions on ordering AmnioFix.
